Complex Implant Cases and Referrals in Cheshire

For patients who have been turned away elsewhere, told their case is too complex, or whose previous implant treatment has failed. And for dental professionals looking for a trusted clinician to refer their most challenging cases to.

If Another Dentist Has Said Your Case Is Too Difficult, That Is Not Necessarily the End of the Story

There is a significant difference between a case that is genuinely not possible and a case that is beyond the capability or experience of the clinician who assessed it.

Complex implant cases require more than the baseline implant training that most dentists hold. They require postgraduate-level education in surgical anatomy, implant biomechanics, and restorative outcomes. They require experience with cases that have failed, cases where bone is compromised, cases where the anatomy is unusual, and cases where previous treatment has left behind problems that need to be addressed before anything new can be placed. They require the kind of clinical judgement that comes only from years of focused, specialist implant work.

Dr Nikhil Oberai has spent his career building exactly that level of expertise. He holds an MSc in Implant Dentistry from the University of Manchester, carries a 99.5% success rate across hundreds of cases, teaches implant dentistry at postgraduate level, and is the clinician that dental practices across Cheshire and the wider North West send their most demanding cases to.

If you have been turned away, told your bone is insufficient, told a previous failure means implants are no longer an option, or simply feel that you have not yet had a thorough enough assessment, a consultation with Dr Nik is the right next step.

The Situations That Require a Higher Level of Implant Experience

Complexity in implant dentistry arises from a range of clinical factors. Some are anatomical. Some are the result of previous treatment. Some are systemic. Understanding what makes a case complex helps explain why not every implant clinician is equipped to handle every case.

Severe bone loss

When significant bone has been lost in the jaw, conventional implant placement may not be straightforward. The volume, density, and location of remaining bone determines what is possible. Cases with severe bone loss may require bone grafting, zygomatic implants, or a carefully planned combination approach to achieve a stable, long-term result.

Failed or failing implants

When an implant has failed or is failing, the bone at and around the failed site is typically compromised. Assessing what caused the failure, treating the site appropriately, and planning any future implant placement requires a thorough understanding of why failures occur and how to address the underlying factors.

Insufficient bone for conventional placement

Bone deficiency does not always require zygomatic implants. In many cases the bone can be augmented through grafting procedures, sinus lifts, or ridge preservation techniques. Determining which approach is appropriate, and planning it correctly, requires experience with a wide range of augmentation techniques.

Complex restorative requirements

Some implant cases are complex not because of the surgery but because of the restorative challenge. Patients with unusual bite patterns, extensive tooth wear, skeletal discrepancies, or previous poorly designed restorations need a clinician who understands both the surgical and the restorative side of implant treatment and can plan both together from the outset.

Patients with significant medical history

Certain medical conditions and medications can affect implant healing and long-term integration. Patients with conditions including uncontrolled diabetes, osteoporosis, or those on certain bone-affecting medications require careful assessment before implant treatment is recommended. Dr Nik will always review your full medical history as part of the consultation process.

Multiple failed previous treatments

Patients who have had multiple treatments fail, whether implants, bridges, or other restorations, often arrive sceptical and with a complicated clinical picture. Approaching these cases requires patience, a thorough diagnostic workup, and a careful plan that addresses the reasons previous treatments failed before placing anything new.

You Are Entitled to a Second Opinion, and It Is Always Worth Getting One

Many of the patients Dr Nik sees for complex cases arrive after having been turned away or advised against treatment at one or more other practices. In a significant number of those cases, treatment turns out to be possible.

That is not a criticism of those other clinicians. Implant dentistry exists on a wide spectrum of difficulty and experience. A practice that offers implants as one of many treatments alongside general dentistry is not equipped in the same way as a clinician who has pursued dedicated postgraduate training and spent years focused specifically on this area. Referring a complex case to someone with more experience in that area is actually the responsible clinical decision, and many good dentists do exactly that.

What matters for patients is understanding that being turned away at one practice does not mean being turned away everywhere. If you have been told your case is too complex, too risky, or simply not possible, come and speak to Dr Nik before you accept that as a final answer.

A second opinion consultation is free, thorough, and completely without obligation.

A Trusted Referral Clinician for Complex Implant Cases Across the North West

Dr Nikhil Oberai welcomes referrals from dental practices across Cheshire and the wider North West for patients whose implant needs exceed what can be offered in a general dental setting.

Referring a patient to Dr Nik means referring them to a clinician who holds an MSc in Implant Dentistry from the University of Manchester, carries a 99.5% success rate across hundreds of cases, teaches implant dentistry at postgraduate level, and has extensive experience across the full range of surgical and restorative implant complexity. It also means referring them to a clinician who will communicate with you throughout the treatment process and ensure your patient is looked after to the standard your own practice would hold.

We understand that referring to a patient reflects trust, and we take that seriously.

Cases We Accept Referrals For

For patients who require a complete arch reconstruction and whose case demands the level of surgical and restorative experience that full arch implant treatment at this level requires.

For patients with severe upper jaw bone loss where conventional implants are not possible and the case requires a clinician with specific training and experience in zygomatic techniques.

  • Complex Bone Grafting and Augmentation

For patients whose bone deficiency requires grafting, sinus lifting, ridge augmentation, or other bone building procedures before or alongside implant placement.

  • Failed and Failing Implants

For patients whose implants have failed or are failing and who need a thorough reassessment of the situation, removal where necessary, and a plan for what comes next.

  • Complex Single and Multiple Implants

For patients where the anatomy, bone volume, restorative requirements, or medical history makes placement more demanding than a straightforward single tooth case.

  • Second Opinion Assessments

For patients who have received a treatment recommendation elsewhere and whose referring dentist or the patient themselves would benefit from an independent second opinion from a clinician with postgraduate-level implant expertise.

How to Refer a Patient to Dr Nikhil Oberai

We aim to make the referral process as straightforward as possible for referring practices.

Submit a Referral

Complete our online referral form or call either practice directly to discuss the case. Please include relevant clinical information, any existing x-rays or imaging, and a brief summary of the treatment history and the reason for referral. The more information you can provide at this stage, the more useful Dr Nik’s assessment will be for your patient.

Patient Contact

Our team will contact your patient to arrange a consultation appointment at whichever of our two Cheshire practices is most convenient for them, either Croft Dental in Wilmslow or Brunner Court in Northwich.

Assessment and Treatment Plan

Dr Nik will carry out a thorough clinical assessment, including any additional imaging required. He will produce a detailed treatment plan and communicate his findings and recommendations back to the referring practice.

Treatment

If the patient decides to proceed, treatment is carried out by Dr Nik personally. Progress updates and completion reports are shared with the referring practice throughout.

Patient Return

Where appropriate, patients are returned to the referring practice for ongoing care following completion of implant treatment. We are committed to a co-operative relationship with referring practices and will always respect the patient’s existing relationship with their own dentist.

The Clinician Behind the Referrals

Dr Nikhil Oberai holds an MSc in Implant Dentistry from the University of Manchester, carries a 99.5% success rate, and teaches implant dentistry at postgraduate level. He practises across Heritage Smile Group’s two Cheshire locations in Wilmslow and Northwich, and accepts referrals from practices across the region for complex and demanding implant cases.

What Sets Dr Nik Apart as a Referral Clinician:

Postgraduate Level Training

Dr Nik’s MSc in Implant Dentistry from the University of Manchester is not a short course. It is a multi-year, clinically assessed programme that represents the highest level of implant qualification available in the UK. The depth of knowledge and surgical experience it requires underpins everything he does at the chair.

That figure reflects consistency across a large and varied case load. It is the result of meticulous planning, precise execution, and a commitment to following each case through to its long-term outcome rather than simply placing implants and moving on.

As an Honorary Clinical Teaching Fellow at the University of Manchester, a mentor at the Cambridge Implant Academy, and an external examiner for the BAIRD Implant Course, Dr Nik is actively involved in shaping the standards to which other implant clinicians are trained. That involvement keeps his own practice current, scrutinised, and evidence-based.

We know that referring practices need to feel confident that their patients are in good hands and that they will be kept informed throughout. We provide written reports following assessment and on completion of treatment, and we are always available to discuss a case with the referring clinician directly.

Patients Who Came to Us When Others Said It Could Not Be Done

“I was referred to Dr Nik by my own dentist who was honest enough to say that my case was beyond what he was comfortable with placing himself That honesty meant a lot, and Dr Nik more than justified the referral. The treatment was thorough, well planned, and the result has been excellent.”

Confused? Let us help

Complex implant cases are typically those involving significant bone loss, failed previous implants, unusual anatomy, or medical factors that affect healing and integration. If you have been turned away at another practice, told bone grafting is required before implants are possible, or had a previous implant fail, your case likely falls into the complex category. A thorough assessment with Dr Nik will give you a clear picture of exactly what you are dealing with and what the options are.

Dr Nik will always be honest about what is possible. If a case genuinely falls outside what can be achieved safely and predictably with implants, even at this level of experience, he will tell you that clearly and explain the alternatives available to you. What he will not do is proceed with treatment that he does not believe will deliver a reliable long-term result.

That depends entirely on the nature and extent of the complexity. Cases involving bone grafting prior to implant placement add six to twelve months to the overall timeline. Cases where zygomatic implants allow immediate fixed teeth can be completed more quickly than a staged grafting approach. Dr Nik will give you a realistic timeline as part of your treatment plan following the initial assessment.

You can self-refer directly. You do not need to be referred by your own dentist to book a consultation with Dr Nik. Simply call either practice or use the online booking system to arrange a consultation appointment. If you do have a referring dentist, letting us know at the time of booking helps us gather any relevant clinical information in advance.

Yes, if you would like him to. With your permission, Dr Nik will communicate assessment findings and treatment progress with your own dental practice. Many patients appreciate this co-ordination and it is always done with the patient’s knowledge and consent.
